如何申请学术住宿
The process to request academic accommodations includes, 但可能不限于, 这五个基本步骤:
- The student submits a completed confidential 访问服务 Intake Form along with current (not more than 3 years old) corresponding documentation to the director of access services.
- Once the Intake Form 和 the appropriate documentation are received, the College’s 访问服务 Committee meets to review the requested accommodations.
- The student is notified of the Committee’s decisions. If the accommodations are approved, the student is given the opportunity to sign a release form. 一旦签署了释放表格, the student will be provided a letter indicating that he or she is requesting the approved accommodations. The student will then provide this letter to the course instructors. Once the faculty member receives the letter, a discussion with the student about the best means by which the requested accommodations will be provided is warranted.
- The student keeps one copy of the letter 和 the faculty member keeps one copy of the letter to use when discussing or implementing the accommodations requested by the student.
- The faculty member submits the on-line proof of receipt to the Ability First Resource Center. If accommodation-specific guidance is needed, faculty members or students should contact Dr. 丽莎·奥莱尼克·多尔曼, dean of academic enrichment 和 504/ADA coordinator, at (334)833-4465 or ldorman@hawks.十大菠菜靠谱平台.edu
This process repeats during subsequent semesters.
监考考试
A student who has approved academic accommodations that require alternative testing procedures, 特别的帮助, 或者一个非传统的工作空间, may decide with his or her faculty member to have an exam, 测试, or other assignments proctored in the Ability First Resource Center (AFRC). An Academic Accommodations Use Form for Examination 和 Testing Services will be completed by the instructor 和 an appointment must be made at least one week prior to the testing date by the student/instructor with the examination coordinator to have the exam proctored.

The 学术充实主任 和504/ADA协调员 is the intake coordinator for students with disabilities who request services 和/or accommodations to minimize the effects of their disabilities in the academic environment. Students must identify themselves voluntarily 和 provide current, official documentation of 残疾 to become eligible for reasonable accommodations. 如果文件不充分, the student may be asked to provide additional information/evaluation.
